At Shenton Park General Practice, we are now offering an Advanced Care Planning Clinic.
This clinic provides a supportive space to discuss your wishes for future health and end-of-life care. You may also create or update a formal Advanced Health Care Directive, ensuring your preferences are documented and respected.
What to Expect
- Most patients will require two appointments (around 20 minutes each).
- After your first appointment, you will be given reading material and tasks to complete before returning for the second session.
Fees
- Standard appointments: $125.00
- Commonwealth concession card holders: Bulk billed
